Our team of Certified Care Managers are a trained Behavioral Health Nurse and a Doctor of Physical Therapy who are highly skilled in the delivery of senior geriatric care. Our specialty is Dementia/Alzheimer’s, Behavioral healthcare and Rehabilitation medicine. We work closely with our clients, their families and their interdisciplinary care team to create and implement the most effective and comprehensive plan of care available. C&V will advocate for your loved one. Our goal is to assist the family in navigating through challenging decisions for their loved one to receive the best possible care and develop a plan for the future. With extensive collaboration we coordinate and monitor our client’s progress to ensure that the client is adhering to their doctors’ recommendations, receiving nutritious meals, taking prescriptions properly, attending medical appointments, and are receiving the support needed to maintain their health. An important part of our collaboration is to help reconcile an individual’s medications to minimize medication interaction and complications.

Comprehensive assessment to develop a patient-centered care plan.

Define client goals and strategies to achieve goals.

Assistance for families caring for their aging parents or loved ones to identify, problem solve, and plan for their needs.

Provide Home Safety evaluation, recommend home modifications to assist them to remain safely at current level of care.

Functional assessments including gait, balance and strength.

Serve as advocate at hospitals , nursing homes, rehab facilities for families whose loved ones are out of town.

Medication reconciliation

Accompany clients/family on visits.

Coordinate transitional care services from the facility, hospital or home including transportation.

Schedule appointments with health care providers based on the client’s care plan.

Refer patients to experts based on their financial, legal, and health care needs.

Educate clients and their families on relevant health education and health services topics.

Conduct routine check-ins to monitor client.

Identify community resources available to clients based on their specific needs.
